You don’t have to live nearby our private clinic in Oskaloosa, Iowa. The same expert care (minus in-person exams unless you visit us!) is available to you in Florida, Virginia, Arizona, California, Michigan, and Iowa too. Enrollment: $100 for a family, husband and wife, or individual
Per month:
$85 for an individual
$135 for a husband and wife
$166.50 for a husband, wife, and children (children 18 years and older $35/month)
Metabolic Health (includes weekly onramp and monthly coaching visits)
Initial consultation and enrollment: $150
Per month: $65, free to direct primary care subscription members
Testosterone Replacement Therapy:
Without primary care: starts at $100/month
